Losing GLP-1 coverage? How to protect your weight loss
- Fewer employers cover GLP-1s for weight loss: one large survey found coverage fell from 72% of employers in 2025 to 60% in 2026, and more plan to drop it for 2027.
- Many plan changes take effect on January 1, so people often have a few weeks' warning during open enrollment.
- Cash prices have fallen in 2026, and Medicare has a $50/month bridge program for eligible members. Your options are worth a conversation with your prescriber before your last covered fill.
- Whatever you decide, the weeks around a forced change are when a plan matters most. Pick your line, weigh daily and have your food-noise plan ready.
If you've just read an open-enrollment letter saying your plan won't cover GLP-1s for weight loss next year, you're not alone. Employers have been pulling back as costs climbed, and many changes take effect at the new plan year.
What's changing
- In the Business Group on Health's survey of large employers, coverage of GLP-1s for weight loss fell from 72% in 2025 to 60% in 2026, and a further group plans to drop it by 2027.
- Some employers keep coverage for diabetes but drop it for weight loss, or add stricter rules such as prior authorization or BMI thresholds.
Options to talk through with your prescriber
We don't give medical or dosing advice, but these are the questions people commonly bring to their appointment:
- Self-pay prices. Manufacturers and the government's TrumpRx site now list cash prices well below the old list prices, and the newer pills start lower than injections. Prices vary by product and dose, so check current figures.
- Medicare. Since July 2026, eligible Medicare Part D members can get certain GLP-1s for a $50 monthly copay through the GLP-1 Bridge program, which runs through 2027.
- A lower dose or a pill. 2026 trials looked at stepping down to a lower dose and at switching to a daily pill. See our guides on maintenance doses and switching to a pill.
- Stopping. If you stop, it helps to know what the trials show and to plan for it. Start with what happens when you stop a GLP-1.
A 12-week plan around the change
- Record your number now. Your 7-day average in the last covered month is your reference.
- Choose your line. For example, 3 lb above your reference average. Write it down.
- Weigh every morning, judge weekly. Small changes are easy to fix and easy to miss.
- Prepare for appetite returning. Whether you stop, step down or switch, hunger and food noise may rise. Write your three-step plan for loud moments while you're calm.
- Keep protein and strength in every week. They help with fullness and protect muscle.
Kept was built for this moment: a 20-second morning check-in, Kept % (how much of your loss you've kept) and an early warning while a gain is still small.
Common questions
My employer is dropping GLP-1 coverage. What should I do first?
Note the date your coverage ends, book a conversation with your prescriber before your last covered fill, and record your current weekly average weight. Having a reference number and a plan in place before the change makes it much easier to act early on any regain.
Are there cheaper ways to get a GLP-1 without insurance?
Cash prices fell during 2026 through manufacturer programs and the TrumpRx site, and the newer pills start at lower prices than injections. Prices depend on the product and dose and change often, so check current prices and talk to your prescriber.
Does Medicare cover GLP-1s for weight loss now?
Since July 1, 2026, eligible Medicare Part D members can get certain GLP-1s for a $50 monthly copay through the GLP-1 Bridge program, which runs through the end of 2027. Eligibility rules apply.
Will I regain weight if I lose coverage and stop?
Many people do regain some weight after stopping. Trials found about half to two-thirds of the loss returned within a year on average, with wide variation. Early warning and steady habits make the biggest difference.
