Coming off Ozempic or Wegovy

By the Kept team · Updated · Sources linked below

Key points
  • Ozempic and Wegovy are both semaglutide. Wegovy is the version approved for weight management.
  • In the STEP 1 extension, people regained about two-thirds of their lost weight in the year after stopping, on average.
  • In STEP 4, people switched to placebo regained 6.9% of body weight over 48 weeks.
  • Appetite usually returns over the first few weeks. Catching drift early, at 2 or 3 lb, is the habit that matters most.

Ozempic and Wegovy contain the same medicine, semaglutide. If you're coming off either one, whether by choice, because of cost or supply, or on your prescriber's advice, here's what the research says about the months after, and what you can do about it.

What the semaglutide trials found

Both trials stopped the medicine at once, and in STEP 1 the lifestyle support stopped too. They tell us what happens when nothing replaces the medicine, which is exactly the gap a maintenance plan is meant to fill.

When appetite comes back

Semaglutide is taken weekly and leaves the body gradually, so many people notice hunger and food noise creeping back over the first few weeks rather than overnight. Bigger portions start to feel normal again. This usually happens before the scale shows much, which makes those early weeks the best time to set up your routine.

A maintenance plan for after semaglutide

  1. Record your starting point. Note your 7-day average weight in the week you stop.
  2. Set your line. Pick the gain that will make you act, for example 3 lb above that average.
  3. Weigh most mornings, judge the weekly average.
  4. Write your food-noise plan now, while appetite is still quiet.
  5. Keep protein at each meal and strength training in your week.
  6. Act at the line with one small change for two weeks.

Tapering, side effects and restarting

Questions about tapering, symptoms after stopping, or restarting belong with your prescriber. We don't give advice on doses or medication decisions.

Common questions

Will I gain weight after coming off Ozempic or Wegovy?

Most people regain some weight. In the STEP 1 extension, people regained about two-thirds of their lost weight on average in the year after stopping semaglutide, but individual results varied widely.

How long after stopping Wegovy does hunger come back?

Semaglutide leaves the body gradually, so many people notice appetite and food noise returning over the first few weeks. It usually comes back before the weight does.

How do I keep the weight off after Ozempic?

Weigh most mornings and watch your 7-day average, set a gain line such as +3 lb, plan ahead for food noise, keep protein at every meal and act early with one small change when you cross your line.
